Take the exit onto I-55 South towards Memphis.

Take exit 6A on the left for I-240 E towards Nashville/I-240 N.

Keep left at the fork and merge onto I-240 E.

Take exit 15A to merge onto US-72 E/Poplar Ave toward Germantown.

There is an entrance off Poplar just east of Briarcrest or you may turn left onto Briarcrest then a quick right into the parking lot.

Take exit 16A for Tennessee 177 S toward Germantown.

Merge onto N Germantown Pkwy.

Continue S Germantown Rd.

Turn right onto W Farmington Blvd.

Take the 1st right onto Poplar Ave.

There is an entrance off Poplar just east of Briarcrest or you may turn right onto Briarcrest then a quick right into the parking lot.

Merge onto I-55 N/I-69 N towards Memphis.

Take exit 6A to merge onto I-240 E towards Nashville/I-240 N.

Keep left at the fork and merge onto I-240 E.

For additional questions

For the best possible experience, please complete the required forms at your convenience prior to your visit. Click here to access the patient portal to access and fill out your paperwork. You will need an access code if you have never logged on before. Please call (901) 624-3333 and press 1 to obtain your access code.

If you prefer paper, please plan to arrive approximately fifteen minutes prior to your scheduled appointment, so we can complete your paperwork before your visit.

We are happy to renew or refill routine prescriptions during our regular office hours as deemed appropriate by our physicians. For compliance with best medical practices and for your health and safety, you will need to be seen in the office for refills if you have not been seen by one of our physicians in the last year for most topicals and in the last 6 months for most oral medicals. In some cases, you may need to be seen prior to having a medication refilled or renewed regardless of the time of your last office visit.

Our office staff is available to answer patient questions in person or by phone during our regular office hours. Since we work through lunch and take very few breaks while taking care of our patients, phone calls will usually require leaving a message on our voice mail system. Voicemails will be checked twice daily at noon and 3 pm. Any calls made after 3 pm will be returned the next business day.

Please be patient with us as we will do our best to return your call as soon as possible! If you have a medical emergency, please call 911 or go the nearest emergency department.
