How Does Asclera® Sclerotherapy Work?

Sclerotherapy is a simple procedure that treats and reduces the appearance of spider veins, which most often appear on thighs, calves, and ankles.

Your dermatologist will inject Asclera® (polidocanol), a sclerosing solution into the dysfunctional vessels. The targeted veins immediately start to shrink, dissolve, and fade from view as the body naturally reabsorbs them. This procedure may also remedy the aching, burning, and swelling associated with spider veins.

Asclera® treats two types of veins: uncomplicated spider veins (very small varicose veins ≤ 1 mm in diameter), and uncomplicated small varicose veins (1 to 3 mm in diameter), also known as reticular veins.

Asclera® is injected directly into the vein to damage the endothelium, the cells lining the inside of blood vessels.

This causes blood platelets to attach to the lining of the vessels. Eventually, cellular debris and platelets cause the blood vessel to clot. Over time, the clotted vein will be replaced with tissue.

You should tell your doctor about all the medicines you are taking, including prescription and nonprescription medications, vitamins, and supplements. You should not be treated with Asclera® if you:

Have a known allergy to polidocanol
Have an acute vein or blood clotting (thromboembolic) disease
Are pregnant or nursing

The treatment itself is no more difficult than having a few injections. We offer a topical numbing solution to maximize your comfort and make sclerotherapy treatment as easy as possible, but most patients find it not necessary to numb. A typical sclerotherapy session lasts 15 to 45 minutes. One injection is usually administered per group of vessels with multiple injections per session. Two to three days following treatment you should wear compression stockings continuously. Then wear them during the day for the next two to three weeks.

For two to three days following the treatment, avoid:

  • Heavy exercise
  • Sunbathing
  • Long plane flights
  • Hot baths or saunas

 

Side effects, although rare include:

  • Severe allergic reaction following polidocanol use, including anaphylactic reactions which can be fatal. Most reactions occur with greater than 3mL volume of medication used, so Levy Dermatology keeps your dosage as low as possible.
  • Small “burns” or ulcers, usually due to Asclera® leakage into the skin. It heals in time, but may scar.
  • Inadvertent perivascular injection which causes pain. If severe, a local anesthetic may be injected.

Patients were significantly more satisfied with Asclera® than with either sodium tetradecyl sulfate (STS) or placebo (p<0.0001).
